Support Services

We don’t do for, we do with.

At Juniper Home Care we don’t want to take away your independence. We’re part of your team, supporting you along the way where you need it. That’s why when we deliver services, we do it in partnership with you so you can keep or regain independence.

Your Personal Care Plan

When you sign-up with Juniper, your Care Lead will visit you in your home to learn all about you and what you want to achieve. Together we discuss and develop a Care Plan that will pull together your team of Juniper professionals so you can stay independent at home and achieve your goals.

That Care Plan, which will change with you as your needs change, can access any of the below Juniper services within your budget so you can get the most from your Home Care Package.

Home Support

  • Personal care: Support with showering, dressing, continence, personal hygiene and beauty routines.
  • Meal planning: Meal planning and preparation, for meals that taste delicious and provide the nutrition you need.
  • Cleaning and household tasks: Support with cleaning, laundry, ironing, changing linen, pet support.
  • Home Safety: Environment safety assessments to identify and remove potential risks and minimise injury and accidents.
  • In-home respite: Providing support to customers so carers can take time for themselves.
  • Light Gardening: Our sub-contractors can keep your garden in check to ensure your safety.

Clinical Support

  • Nursing: Medication management, continence support, wound care, catheter care, comprehensive health assessments and much more.
  • Physiotherapy: maintain strength, assist rehabilitation, support independence and improve general wellbeing.
  • Occupational therapy: Falls prevention, health aids and equipment, home modifications to maintain safety and independence, cognitive screening.
  • Podiatry: Assessments and care plans, checking skin integrity and pressure points, nail care, managing foot pain and more.
  • Group therapy: Classes run by physiotherapists and therapy assistants to build strength, recovery, regain mobility and more.
  • Dementia Care: Specialised services dedicated entirely to the specific needs of dementia care, provided by experienced at home care staff.
  • Palliative care: Palliative care with dignity, respect and empathy at Juniper is an inclusive response to end of life care.

Social Support

  • Companionship: Fancy a game of cards or love a good chat? Our friendly team can support you with doing the activities you enjoy most.
  • Transport: Transport services for appointments, shopping, errands and social visits, helping you to stay connected.
  • Shopping: From your grocery shop to popping into the chemist.

Trying to manage her diabetes, Joan was struggling and becoming increasingly anxious about her blood sugar levels. Juniper Home Care’s nursing team provided education to Joan and her husband for three months, equipping Joan with the tools she needed to keep her diabetes in check and to regain her confidence. As Joan’s confidence and knowledge grew, Juniper nurses gradually visited less and now Joan is managing independently. Did we at Juniper do ourselves out of a job? Absolutely. But that’s exactly what Joan wanted –to be independent at home, with the tools and knowledge to manage herself.

Dianne was finding it hard to move around and was feeling fearful to clean her home the way she always had. Now Dianne teams up with her Juniper support worker Sonam. Dianne dusts and wipes the bench. Sonam vacuums and mops. Dianne now loves having her family over in her sparkling home. Do we do all Dianne’s cleaning? No way, Dianne can do plenty herself. We provide the support on those tasks that are out of reach and Dianne keeps her fitness and wellbeing up doing the tasks she’s more than capable of. Together they make a great team.
