When you sign-up with Juniper, your Care Partner will visit you in your home to learn all about you and what you want to achieve. Together we discuss and develop an in-home Care Plan unique to you with the right mix of supports to suit your needs.

Your approved My Aged Care assessment determines which services you can access to support your lifestyle and goals.

Nursing: Medication management, continence support, wound care, catheter care, comprehensive health assessments and much more.
Physiotherapy: maintain strength, assist rehabilitation, support independence and improve general wellbeing.
Occupational therapy: Falls prevention, health aids and equipment, home modifications to maintain safety and independence, cognitive screening.
Podiatry: Assessments and care plans, checking skin integrity and pressure points, nail care, managing foot pain and more.
Group therapy: Classes run by physiotherapists and allied health assistants to build strength, recovery, regain mobility and more.
Dietitian or Nutritionist: nutritional assessments, meal plans to manage chronic diseases and improving quality of life.
Care Management: Care planning, service coordination, monitoring, review and evaluation, support and education.
Clinical Care Management: planning, monitoring, and continuously adapting your care plan to meet your changing needs, focusing on promoting safety, independence, and quality of life through ongoing assessment, service coordination, and regular care reviews.
Personal care: Support with showering, dressing, continence, personal hygiene and beauty routines.
Social Support and Community Engagement: Companionship and supporting you to connect with activities and your community.
Transport: Transport services for appointments, shopping, errands and social visits, helping you to stay connected.
Respite: Providing support to customers so carers can take time for themselves.
Remedial masseuse: Therapeutic massage to treat soft tissue injuries, musculoskeletal problems, and chronic pain. in check to ensure your safety.
Shopping: From your grocery shop to popping into the chemist.
Domestic Assistance: Support with cleaning, laundry, ironing, changing linen, pet support.
Shopping: Your shopping from your list.
Meal preparation: For meals that taste delicious and provide the nutrition you need.
Gardening: Our sub-contractors can keep your garden in check to ensure your safety.

Day Therapy Centres in Bentley and Karrinyup support your health and well-being with access to allied health and nursing.Day Therapy Centres in Kununurra and Wyndham offer an inclusive and welcoming environment where you can feel safe and happy and stay connected with your local community with a range of services likes meals and transport and a whole range of activities.Juniper offers services under Support at Home’s Assistive Technology and Home Modifications (AT-HM) scheme.
If you have been approved for this Scheme by your My Aged Care assessment, you can access a comprehensive range of assistive technologies and home modifications designed to enhance your safety, independence, and quality of life.
Our Care Management service takes care of all the complex coordination so you can focus on living well. Care Management is set at 10 per cent which will be deducted from your quarterly budget and held by Services Australia.

• Scheduling and coordinating your services.
• Coordinating between your care workers, health professionals, and service providers.
• Quality assurance with continuous monitoring of safety and service standards.
• Connecting you to additional community resources where needed.
