What is the Transition Care Program?

The Juniper Transition Care Program (TCP) provides short term care for up to 12 weeks for those who need additional support after staying in hospital. You can receive this care in a residential home, your own home, or a combination of both. This goal-orientated program looks to improve your health and mobility, so you can continue to live independently at home. If you need longer term support and care, the Transition Care Program lets you explore future living arrangements. Juniper will collaborate with you to achieve the best outcomes. We have multi-disciplinary teams to meet your wellbeing goals.

Transition Care Program Services in Perth and Albany
Medical Support, such as nursing.

  • Medical Support, such as nursing.
  • Social Support through a social worker and case manager.
  • Therapy Support, like low intensity physiotherapy and occupational therapy.
  • Personal Care, such as showering.
  • Domestic Assistance, such as cleaning and meal preparation.
  • Transport to and from medical appointments (subject to eligibility, speak with our team to discuss).

If you are in a Juniper Residential Aged Care home or Juniper Karrinyup TCP, you will have your own bedroom with an ensuite and meals provided.

How to access Transition Care

You can access Juniper’s Transition Care Program across Perth through a referral from the Aged Care Hub.

Transition Care Program – fees and costs

A Basic Daily Fee is payable for those who can afford to do so. Inability to pay fees will not affect access to the Program. Juniper determine your capacity to pay the Transition Care Program fees and costs. We will consider any exceptional and unavoidable expenses incurred by you through a Juniper financial hardship application.

Meet TCP Navigator John Sillah

John brings a valuable mix of experience to his navigator role. He has worked as a community carer, a Care Partner, and later as a social worker in palliative care after completing his social work degree.

“My social work background and Home Care experience really support the navigator role – especially when it comes to troubleshooting and care planning,” John said.

John discovered his passion for aged care while working as a carer, realising he had a natural ability to connect with people and build strong relationships.

Over the past two years as a navigator, John has supported customers both in the community and at Juniper’s Annesley and Hayloft sites in Bentley and Martin.

“There’s great satisfaction when someone shares how they’re feeling and you’re able to provide reassurance and guidance,” he said. “It really feels like you’re making a difference.”

Meet TCP Navigator Gerard Quigan

It’s important that customers see Gerard as genuine, approachable and solution-focused.

“I enjoy solving problems and thinking outside the box,” Gerard said. “While I understand the clinical side, I don’t come from a clinical background, which means I can explain things in a way that’s easy for customers to understand.”

Gerard joined Juniper three years ago as a Care Partner and moved into the Navigator role a year ago. He now supports customers in the community and at Juniper’s Karrinyup and Albany sites.

Before entering aged care, Gerard spent 37 years as a chef in New Zealand. After moving to Tasmania to be closer to his ageing in-laws, he began working as a Community Support Worker.

He later became a Care Partner, where he loved building relationships and seeing the difference he could make in people’s lives.

Now working in TCP, Gerard enjoys the program’s short timeframes and holistic approach.

“The support is intensive, but the results are often fast – physically and mentally. Seeing people regain confidence so quickly is incredibly rewarding,” he said.
