Support at Home (formerly a Home Care Package) is available to Australians aged 65 years and over and is designed to help older people get the support they need to stay in their own homes. The government provides funding to pay for services which are personalised based on what each person needs. The program aims to help seniors live comfortably and independently at home for as long as possible.
To receive Support at Home a person needs to be assessed by My Aged Care. An Assessment is an individual assessment to determine the level of support and services each person may need to remain at home.
Under Support at Home there are eight ongoing funding classifications. The more care you need, the higher your classification will be. This could be as simple as weekly shopping and cleaning services to more complex daily support such as nursing, physiotherapy, and carer support to enable you to remain at home.
Along with the eight ongoing funding classifications, there are three short term classifications including the Restorative Care Pathway; the End-of-Life Pathway; and The Assistive Technology and Home Modifications (AT-HM) scheme.
When you receive a letter from My Aged Care informing you that your funding has been approved, you choose the Home Care provider, like Juniper, that meets your individual needs.
Juniper will partner with you to put together a Care Plan, drawing from your approved home support services that have the flexibility to maintain your independence and wellbeing, as you continue to live your life.
