Commonwealth Home Support Programme

A Commonwealth Home Support Programme (CHSP) with Juniper is all about keeping you independent at home, enjoying what you love in life, connected to your community.

CHSP is an entry-level programme for people 65 years or older (50 years or older for Aboriginal or Torres Strait Islander people) who can manage at home, but just need a bit of support for some tasks.

If you’re experiencing trouble with a few everyday tasks and feel that a little support could improve your health and wellbeing, then CHSP could be the perfect solution. It is also great as a short-term solution, perhaps after a hospital stay or recovering from an illness.

How much does the Commonwealth Home Support Programme cost?

The Government will fund the cost of your services but it is expected CHSP providers, like Juniper, charge customer contributions where customers can afford to do so. Juniper has set a competitive co-contribution fee to ensure it covers the cost of delivering services.

What Juniper Home Care services can I access with CSHP?

Any of our Juniper in-home care services are available under CHSP. This could be help with tasks such as showering and getting dressed, cleaning and housework, support with social outings and transport.

The program also provides personalised clinical health care services such as nursing, physiotherapy, occupational therapy and podiatry via one of Juniper’s two metropolitan Day Therapy Centres (North, South) or if eligible, in the comfort of your own home.

Home Support

  • Personal care: Support with showering, dressing, continence, personal hygiene and beauty routines.
  • Meal planning: Meal planning and preparation, for meals that taste delicious and provide the nutrition you need.
  • Cleaning and household tasks: Support with cleaning, laundry, ironing, changing linen, pet support.
  • Home Safety: Environment safety assessments to identify and remove potential risks and minimise injury and accidents.
  • In-home respite: Providing support to customers so carers can take time for themselves.
  • Light Gardening: Our sub-contractors can keep your garden in check to ensure your safety.

Clinical Support

  • Nursing: Medication management, continence support, wound care, catheter care, comprehensive health assessments and much more.
  • Physiotherapy: maintain strength, assist rehabilitation, support independence and improve general wellbeing.
  • Occupational therapy: Falls prevention, health aids and equipment, home modifications to maintain safety and independence, cognitive screening.
  • Podiatry: Assessments and care plans, checking skin integrity and pressure points, nail care, managing foot pain and more.
  • Group therapy: Classes run by physiotherapists and therapy assistants to build strength, recovery, regain mobility and more.
  • Dementia Care: Specialised services dedicated entirely to the specific needs of dementia care, provided by experienced at home care staff.
  • Palliative care: Palliative care with dignity, respect and empathy at Juniper is an inclusive response to end of life care.

Social Support

  • Companionship: Fancy a game of cards or love a good chat? Our friendly team can support you with doing the activities you enjoy most.
  • Transport: Transport services for appointments, shopping, errands and social visits, helping you to stay connected.
  • Shopping: From your grocery shop to popping into the chemist.
