Home Care Packages

A Home Care Package with Juniper is all about keeping you independent at home, enjoying what you love in life, connected to your community.

To be eligible for a Home Care Package, you must be 65 years old and over, or 50 years or older for Aboriginal or Torres Strait Islander people. Home Care Packages are for people with greater or more complex care needs who need ongoing care and support services.

How you spend your Home Care Package funding is flexible, with care and services personalised to you to meet your needs and goals, keep you safe and independent at home, and improve your health and wellbeing.

Home Care Package levels

The Government will assign you a level for your Home Care Package depending on your assessed needs. The level corresponds with the amount of funding you receive.

Package levels

Level 1 – Subsidised up to $10,271.10 a year

Level 2 – Subsidised up to $18,063.85 a year

Level 3 – Subsidised up to $39,310.50 a year

Level 4 – Subsidised up to $59,593.55 a year

You can access any of our services from any package level. That means you can access services like physiotherapy, occupational therapy or nursing within the Care Plan you develop with your Juniper Care Lead, no matter your Home Care Package level.

Juniper offers Home Care Services right across Perth, the Peel region, across parts of the Wheatbelt from its Northam base and in the Kimberley from our Day Centres in Kununurra and Wyndham.
