Juniper Charles Jenkins Transition Care, Bentley

The Transition Care Program is a government initiative that provides short term care for older people after a stay in hospital, giving them more time to recover before returning home or moving into a residential care home.

Juniper Charles Jenkins in Bentley is a Transition Care service, designed specifically for this purpose.

Our team of friendly and experienced staff will support you in your recovery and respect your choices as you realise your longer term plans.

From highly trained nurses, carers and allied health professionals such as physiotherapists, to social workers and Pastoral and Spiritual Care workers, you will receive all the personalised care and support you need while you recover and plan your way forward.
