Transitioning into residential aged care

At Juniper, we understand the significance of transitioning into residential aged care. We know that this journey may involve mixed emotions, uncertainties, and a sense of leaving behind a familiar environment. However, we believe in celebrating the uniqueness of everyone, their preferences, relationships, and life experiences. When you decide to call Juniper Residential Aged Care your home, we are committed to supporting you so you can keep being you.

We aim to ease the transition process by ensuring that you feel welcome, respected, and valued every step of the way. We’ve put together a list of tips which may help you as you transition to your new home.

1. Inform the necessary people and organisations about your move.

To help you with this, you can download the My Aged Care checklist, which outlines who you should notify.  We also recommend that you read our Legal and financial planning blog as the more you prepare, the more peace of mind you’ll have as you embark on this new chapter in your life.

2. Don’t do it all alone

While you might be the main contact for the residential aged care home, we recommend that you lean on others for help where you can. Moving into an aged care home isn’t something you should go through on your own. Our team is also available to support you.

3. Keep being you

We understand that you have unique quirks, interests, and aspirations. Our dedicated team will work closely with you, your loved ones, and your healthcare providers to create a care plan that caters to your specific needs, preferences, and goals. Your room will also include a bed and some furnishings, but you have the freedom to personalise your space to make it feel like a home.  Our team will also provide you with a list of approved items to bring.

4. Get involved in social activities

We understand the importance of social connections and the positive impact they have on overall well-being. Our community encourages the development of new friendships and opportunities to engage in a variety of activities and events. Whether it’s participating in hobbies, cultural celebrations, or simply enjoying meaningful conversations, we will help you find companionship and a sense of belonging.

5. Encourage friends and family to visit as normal

We know the vital role that family and friends play in your life. Juniper embraces the involvement of your loved ones, encouraging their active participation in your care and the overall community. We provide spaces and amenities for family visits, outings, and quality time together.

6. Continuity of care

Moving into an aged care home should not disrupt the continuity of your healthcare journey. We collaborate closely with your existing healthcare providers, ensuring a seamless transfer of medical records, ongoing support, and necessary therapies. Your health and well-being will remain our top priority, and we commit to providing high-quality care while maintaining open communication with you and your healthcare team.

7. We are here for you

Transitions and changes in life can bring about a range of emotions and how we respond to these challenges can vary. Our Pastoral and Spiritual Care team bring professional training, skills, and a confidential approach to supporting you in times of need. Our team of compassionate professionals provide an empathetic and caring service that caters to your individual situation.

Transitioning into residential aged care doesn’t have to be overwhelming.
