Juniper has welcomed the WA Government’s announcement of the State’s first Older Adult Health Hub in Cockburn, describing it as an important step forward in strengthening accessible, community-based care for older Western Australians.
Operated by the South Metropolitan Health Service and located within the Cockburn Health and Community Facility, the hub will bring together a wide range of clinical services in one location. Co-located with general practice, pathology, medical imaging and other community services, the hub is designed to make it easier for older people and their families to access the care and support they need.
The Cockburn hub is the first of three sites to be delivered across metropolitan Perth as part of the WA Government’s $23.4 million investment in accessing integrated health services for older Western Australians.
Juniper CEO Russell Bricknell said the initiative aligns closely with Juniper’s focus on supporting older people to live well at home and in their communities.
“Access to timely, coordinated healthcare is essential for older Western Australians, particularly as we head into the colder winter months when health needs often increase,” Mr Bricknell said.
“This type of integrated hub makes it easier for people to connect with the right services sooner, supporting better health outcomes and helping them maintain their independence.”
The hub will provide a broad range of services, including comprehensive geriatric assessments, physiotherapy, nursing care, medication management, dietetics, and memory and cognition support. It will also assist families in navigating the aged care system and connecting with community and social services.
Mr Bricknell said the model represents a positive shift toward more proactive and connected care.
“Bringing multidisciplinary teams together in one location strengthens the delivery of community-based care and ensures older people can access the support they need in a more seamless way,” he said.
The Older Adult Health Hubs are designed to support earlier intervention, helping older Western Australians stay healthier for longer and reducing the need for hospitalisation. By improving coordination across services, the hubs aim to deliver more holistic care that responds to both clinical and social needs.
With Western Australia’s ageing population continuing to grow, Juniper supports initiatives like the Cockburn hub that play an increasingly important role in supporting the wellbeing and independence of older people across the state.
Access to the hubs is available through a referral from a GP and is designed for older adults in the Perth metropolitan area whose health needs are becoming more complex or who require short-term support and care coordination.
Eligibility generally includes people aged 65 and over (or 50 and over for Aboriginal people), living locally, and experiencing changes in health, independence, or support needs.
The hubs are intended for people with more complex needs but do not replace emergency or crisis services. Urgent or acute care should still be sought through hospital emergency departments.
