Assistive Technology and Home Modifications

The Support at Home Assistive Technology and Home Modifications (AT-HM) scheme provides access to equipment, products and home modifications to support you to safely and independently live at home.

If you have been approved for this Scheme by your My Aged Care assessment, you can access a comprehensive range of assistive technologies and home modifications designed to enhance your safety, independence, and quality of life.

What is assistive technology?

Assistive technology are tools that support independence and daily tasks such as walking aids (frames, canes), wheelchairs and mobility scooters, adaptive cutlery, personal alarms to detect falls, and bed sensors.

Our qualified allied health professionals conduct thorough assessments to recommend equipment that matches your specific needs, abilities, and lifestyle preferences.

What are home modifications?

Home modifications transform your home into a safer, more accessible environment tailored to your changing needs. These modifications range from installing grab rails and ramps to bathroom renovations and kitchen adaptations that support your independence.

Our qualified professionals assess your home environment and recommend modifications that reduce fall risks, improve accessibility, and help you continue living comfortably in your own space.

How can I get AT-HM supports?

Your My Aged Care assessment will determine if you need products, equipment or home modifications to help you live safely and independently at home. If you are approved for the AT-HM scheme, funding will be made available based on your needs and included in your support plan.

How is the AT-HM scheme funded?

The AT-HM scheme is funded separately to your ongoing Support at Home services.

There are three funding tiers which cover a health assessment, prescribed items and wrap around services. Items that can be accessed from the scheme must be listed on the government approved list.

The assistive technology funding tiers for 12 months are:

High Up to $15,000*

*High tier assistive technology is not capped at $15,000. Customers who have assistive technology costs above $15,000 can access additional funding with evidence, such as a valid prescription.

The home modifications funding tiers for 12 months are:

*Funding may be extended for additional 12 months to complete complex home modifications if evidence is provided to Services Australia.

If your needs change and you require additional equipment, products or home modifications after your AT-HM funding has expired, we can help you get re-assessed to get access to further AT-HM funding to meet your needs.

Will I have to contribute to my AT-HM costs?

You will not have to contribute to AT-HM wrap-around services as these fall under clinical services.

The administration cost of delivery of assistive technology is up to 10 per cent of the total cost, or up to $500 (whichever is lower). Coordination costs in delivering home modifications is up to 15 per cent of the quoted costs or up to $1,500 (whichever is lower).

AT-HM items will attract a contribution rate under the independence category from 0-25 per cent.

How can I get AT-HM supports if I am moving from the Home Care Packages (HCP) Program?

The AT-HM Scheme was introduced on November 1 with Support at Home, replacing the Home Care Package program.

If you were already on a Home Care Package, you can talk to your Juniper Care Partner about any needs you have for assistive technology or home modifications.

If you have any HCP unspent funds you can use those for assistive technology and home modifications. You must use your unspent funds before you can use the AT-HM scheme funding.
