Your Juniper Home Care team is your Care Lead and might include a nurse, physiotherapist, occupational therapist, therapy assistant and support worker.

You lead your team with your choice and control. You know yourself best and we want to learn all about you. We can then apply our experience, specialist knowledge and skills to what you need to reach your goals.

We take a preventative approach so you can stay healthy. You shouldn’t have to wait for something bad to happen. We want to observe and listen to you to identify early indicators.

Home Care eligibility and funding

How do I apply for Home Care Support Services?

You can contact My Aged Care via their website or call directly on 1800 200 422 to start your application. Your application starts with your telephone conversation with My Aged Care, followed by an assessment in your home to help determine the best level of care or assistance for you.

Juniper can also help you get started with My Aged Care with a guided referral. Just give the friendly team a call on 1300 313 000.

 

Will Juniper help me navigate the Aged Care system so I can get funding to receive Home Care?

What is a Home Care Package (HCP)?

A Home Care Package (HCP) is available to Australians aged 65 years and over and is designed to help older people get the support they need to stay in their own homes. The government provides funding to pay for services which are personalised based on what each person needs. The program aims to help seniors live comfortably and independently for as long as possible.

To get a Home Care Package a person need to have an Aged Care Assessment Team (ACAT) Assessment. An ACAT Assessment is an individual assessment to determine the level of support and services each client may need to remain at home.

Home Care Packages are available in four levels and are designed to assist people with varying needs. This could be as simple as weekly shopping and cleaning services to more complex daily support such as nursing, physiotherapy and carer support to enable you to remain at home.

When you receive a Home Care Package, you choose the Home Care provider, like Juniper, that meets your individual needs.

Juniper will partner with you to put together a Personalised Care Plan under your Home Care Package, drawing from a wide range of home support services that have the flexibility to maintain your independence and well-being, as you continue to live your life.

What costs are involved with Home Care Packages?

Home Care Packages are government-subsidised, however, you may need to make a contribution. How much you will need to pay will depend on your financial situation which the Government will figure out with an assessment of your income and assets (a means test).

The amount of funding you receive through your Home Care Package depends on the level you are assigned following your Aged Care Assessment Team (ACAT) assessment.

What is the Commonwealth Home Support Programme (CHSP)?

CHSP is an entry-level home care program for older people, including short-term or more regular help with tasks such as showering and getting dressed, cleaning and housework, support with social outings and transport.

The program also provides personalised clinical health care services such as nursing, physiotherapy, occupational therapy and podiatry via one of Juniper’s two metropolitan Day Therapy Centres or if eligible, in the comfort of your own home.

The Commonwealth Home Support Program replaced HACC services, from 1 July 2018.

Am I eligible for the Commonwealth Home Support Programme (CHSP)?

To be eligible for CHSP you need to be aged 65 or more years old, or 50 years or more for Aboriginal and Torres Strait Islander people, and require assistance with daily living.

To receive care in the home you will need to apply to My Aged Care and then be assessed by a Regional Assessment Team who will determine your eligibility and advise the services you can access.

What costs are involved with CHSP?

The Government will fund the cost of your services but it is expected CHSP providers, like Juniper, charge client contributions where clients can afford to do so. Juniper has set a competitive co-contribution fee to ensure it covers the cost of delivering services. Click here for fee information.

 

Are the allied health services at Juniper covered by government subsidies?

Yes. You can receive funding through your Home Care Package or Commonwealth Home Support Programme. You can find out more about how to access funding here.

How can I access home care support if English is not my first language?

You can call My Aged Care through their Translating and Interpreting Service (TIS National) on 131 450 or access information on the My Aged Care website in 22 languages.

Juniper Home and Community Care

What does CHSP help at home include?

Depending on what you need, your Juniper Care Lead will work with you to manage your day-to-day with a focus on your independence and safety at home.

You may be recovering from an illness or an operation or simply need some assistance at home for a short or perhaps longer period of time. Juniper can help you achieve your goals in your own home with services such as:

  • Personal care like showering and getting dressed
  • Help at home, working with you to the cleaning and housework tasks you are finding difficult
  • Social activities and transport

The program also has clinical health care services such as nursing, physiotherapy, occupational therapy and podiatry at Juniper’s two metropolitan Day Therapy Centres – or if eligible, in the comfort of your own home.

What do home care services include?

Home Care can include all sorts of services across clinical, home and social support, depending on what your needs and goals are. When you sign-up with Juniper, your Care Lead will visit you in your home to learn all about you and what you want to achieve. That way, together, we can come up with a customised Care Plan that will pull together your team of Juniper professionals so you can achieve your goals, stay independent and keep living independently at home. Some of the services include (but are definitely not limited to):

  • Personal care, like support with showering, dressing, continence, personal hygiene and beauty routines.
  • Cleaning and household tasks like support with cleaning, laundry, ironing, changing linen, pet support.
  • In-home respite to provide support to customers so carers can take time for themselves.
  • Nursing, with medication management, continence support, wound care, catheter care, comprehensive health assessments and much more.
  • Allied health with Physiotherapy, Occupational Therapy, Podiatry and Group classes.
  • Dementia Care, with specialised services dedicated entirely to the specific needs of dementia care, provided by experienced at home care staff.
  • Social Support, like companionship, transport and shopping

Do I get my own Home Care team at Juniper?

Yes. Your main point of contact is your Juniper Care Lead. You will have Juniper staff, for example a support worker, nurse, physiotherapist or occupational therapist, under your Personal Care Plan to meet your goals. Your Juniper team can also refer questions to our Juniper Multidisciplinary Review team of our most senior and experienced Home Care clinical leaders to ensure you are receiving the best and most appropriate support.

Does it matter what level Home Care Package I have to get support from the Juniper Allied Health team?

Not at all. You can access any of our services from whichever levels package you are. That means if physiotherapy, an occupational therapist or podiatrist can support you to reach your goals under your Personal Care Plan, then you can access it on whichever level you’re on.

Can I get Allied Health services, like a physiotherapist, occupational therapist or podiatrist through the Commonwealth Home Support Programme?

Yes! If you have been issued a referral code from My Aged Care for allied health like physiotherapy, occupational therapy or podiatry, our Juniper in-house team of physiotherapists, occupational therapists or podiatrists can support you at one of our Day Therapy Centres North of the River in Karrinyup or South of the River in Bentley. If you are eligible, our Allied Health team can visit you in your own home.

Can I get Allied Health services, like a physiotherapist, occupational therapist or podiatrist through my Home Care Package?

Most definitely. Your Care Lead will work with you to create a Personal Care Plan so you receive the right services to achieve your goals. This might include a physiotherapist, occupational therapist, podiatrist or a combination depending on your needs and what you want to achieve.

How can I access Juniper's Aged Care Allied Health Services or Juniper Nursing Services for myself or a loved one?

Juniper Allied Health services can be accessed through a Home Care Package or Commonwealth Home Support Programme.

What sorts of services can a Juniper Nurse do with Home Care customers?

To support your health and wellness, Juniper Enrolled and Registered Nurses can support you with a many clinical needs, either in the comfort of your own home or through a Juniper Nursing clinic. We can look after clinical needs such as (but definitely not limited to):

  • Wound care and wound reviews
  • Blood pressure and blood glucose assistance
  • Indwelling catheters
  • Stoma care
  • Assessments, such as nutrition, mental health, falls risk, skin integrity and general health and wellbeing
  • And much more

Are Juniper services, like physiotherapy, occupational therapy, podiatry and nursing available for seniors living at home, or only for those in Juniper's Residential Aged Care homes?

Absolutely not! We have a Home Care clinical team of physiotherapists, occupational therapists, podiatrists and nurses dedicated solely to looking after the needs of our Home and Community Care customers. Physiotherapists can help with managing your pains and aches to help you continue to enjoy the activities that you do. Occupational Therapists can assist with home modifications to make access and moving around safer and easier, as well as recommend certain equipment to make your life easier and safer. Plus much more.

Can services, like physiotherapy and occupational therapy, help with pain management and mobility issues in older people?

Yes. If pain management and mobility is something you would like to address we can tailor your personal care plan to encompass this. Physiotherapists can help with managing your pains and aches to help you continue to enjoy the activities that you do.

How do I know if I need a physiotherapist, occupational therapist or podiatrist?

That’s what our team is here for. Your Juniper Care Lead will work with you to create a Care Plan so you receive the right services to achieve your goals. That might mean you want to be able to walk down the street to the park. That means a Physiotherapist might be right for you to work on strength, stamina and balance so you are able and confident to make that park trip. Or maybe you want to be able to catch the bus to get to your favourite activity but the step onto the bus is too daunting? Our Physiotherapist can work on strength and balance, our Occupational Therapist can help find the right aids and our podiatrist can recommend on which footwear would be best to provide you with support and stability whilst you are out.

What are the benefits of a Physiotherapist for Home Care and aged care customers and how can it benefit seniors and help improve quality of life?

Our Juniper Physiotherapists can improve or maintain your strength, balance and stamina so that you can carry on with the activities that you enjoy doing.

We can help with manage your pains and aches to help you continue to enjoy the activities that you do.

We also have group-based exercises that are highly sociable, so while improving strength, balance and stamina you can meet like-minded people.

Our Juniper Physiotherapists can advise and provide exercises to help you reduce any falls or near misses that you may have experienced.

What are the benefits of an Occupational Therapist for Home Care and aged care customers and how can it benefit seniors and help improve quality of life?

Our Juniper Occupational Therapists can assess your home environment and recommend changes so that it optimised for you to carry on with your daily activities safely.

We can advise and assist with putting strategies in place to reduce any falls or near misses that you may have experienced.

We can help you choose the best seating option for your posture, comfort and body shape but also to optimise pressure care so you don’t develop pressure injuries.

Our Juniper Occupational Therapists can assist with home modifications to make access and moving around safer and easier, as well as recommend certain equipment to make your life easier and safer.

What does a Juniper Physiotherapist do with Home Care customers?

At Juniper, we always start our service with an assessment to determine what your needs and goals are. The assessment is made up of a conversation with you and a hands-on assessment. This helps us get to know you, your habits and how the issue at hand affects you. We would use that information to design a tailored program to your needs and to achieve the things you want to do. We may recommend that a therapy assistant complete the exercises with you instead of a physiotherapist if you can complete the exercises with minimal help as this would save you some funds to spend on other things that you may require.

What does a Juniper Podiatrist do with Home Care customers?

Your Juniper Podiatrist will complete a general foot assessment for your initial appointment to determine what is required.

If you a diabetic, this will be a diabetic foot assessment and will include in-depth assessment, such as testing your sensation.

Your Juniper Podiatrist can advise and provide your regular nail care and general foot care, as well as recommend footwear for any issues you may have, such as swollen feet or needing more supportive shoes to reduce falls.

We can look after wounds on your feet at our Karrinyup or Bentley clinics.

Where can I access a Juniper physiotherapist in Perth?

We offer our physiotherapy services in many ways and places, such as in our clinics in Karrinyup and Bentley, group sessions in our therapy centres in Karrinyup, Dianella and Bentley, hydrotherapy in Wilson, and of course physiotherapy in the comfort of your own home. Where your sessions will take place will be discussed in collaboration between yourself and your Juniper physiotherapist following your initial assessment in your home.

What’s the benefit of having an in-house allied health team?

At Juniper having our own Physiotherapists, Occupational Therapists and Podiatrists means we have great communication and collaboration, better efficiency as we can refer internally, and continuity of care. It is also cheaper for our customers to go in-house than source an external allied health service.

How do Juniper’s Physiotherapists, Occupational Therapists and Nurses work together for customers?

Our Juniper clinical team have regular meetings where we discuss different customers’ issues so that we can have the best unified approach to solving an issue. Because we have in-house allied health professionals and nurses, it means you don’t need to shop around if you need to be seen by another discipline as we can refer internally to make it a seamless process.
