Call My Age Care
The Australian Government’s My Aged Care service is the place to go to find out about your eligibility for aged care services.
An initial call to My Aged Care on 1800 200 422 will help determine the right kinds of aged care services that meet your needs and preferences.
Eligibility is determined by an aged care assessment at your home, which can also be arranged with My Aged Care. Once completed, you will receive a letter from My Aged Care to advise:
- The results of the assessment and your eligibility
- The services you can receive and your referral code
- Your support plan
Income and Assets
An important part of applying to aged care is your or your loved one’s personal and financial information.
Knowing your health and personal situation helps Juniper find your new home and tailor the right kinds of services for you.
Financial information enables the Australian Government, through Centrelink, to determine accommodation fees and any subsidies that can support you.
Application and waitlisting
One you have a referral number from My Aged Care and an assets and income assessment from Centrelink your application is even easier.
To help us progress your request as quickly as possible, please complete the application form in full and return it to Juniper.
Once we have received your completed forms, we can confirm your place on Juniper’s wait-list and provide you with an initial accommodation quote for your consideration.

Offer and acceptance
Our application and wait-listing service, Juniper Access, will call you regarding the offer of placement and we can help clarify any queries you have.
We can help you to arrange a viewing of the room and the home’s common areas. For privacy residents’ rooms that are occupied are not available for viewing.
